SEND Transition
How we support children/young people with SEND starting at our school
At Moorhouse Academy pupils are supported with a calm, structured, nurturing environment which instils independence, positive attitudes to learning and caring for one another. School staff work closely with parents to ensure pupils settle in well and for pupils with SEND, the SENDCo ensures a holistic approach to transitions is carried out.
Moorhouse Academy recognises the importance of parental involvement for pupils with SEND and so holds parent coffee mornings, parent workshops and an ‘Open Door’ policy to discuss concerns or raise questions with the SENDCO or class teacher.

How we support children/young people with SEND moving between classes in our school
Thorough transition meetings are held at the end of the academic year for all pupils. This is additionally detailed for learners with SEND. The SENDCO supports these meetings to ensure all information and documentation is passed on. Children with SEND are also provided with additional visits and transition materials to support their move before the summer holiday.
How we support children/young people with SEND leaving our school
Children with SEND are given an extensive level of support before they move to their new school. We work closely with parents and their school of choice to make transition as smooth as possible. As well as having visits from the SEND co-ordinator at the new school, there are opportunities for extra transition visits and usually a named person to act as a point of contact. The new school will be invited to attend SEND review meetings in the summer and all relevant paperwork is passed on.
